Senior FHIR Engineer

Abacus is seeking a FHIR Engineer with strong expertise in healthcare interoperability standards, particularly the FHIR (Fast Healthcare Interoperability Resources) framework. The ideal candidate will have hands-on experience with the technical aspects of FHIR, including authorization, consent management, and secure API implementation. This role requires both a solid understanding of healthcare data exchange standards and the ability to design, implement, and maintain scalable FHIR-based solutions.
Key Responsibilities
• Design, implement, and maintain FHIR APIs in compliance with HL7, CMS, and ONC regulations.
• Configure and manage FHIR servers (e.g., Firely, HAPI, HealthLake) for ingestion, normalization, and access of healthcare data.
• Implement OAuth 2.0 / OpenID Connect / SMART on FHIR for authorization and authentication.
• Support consent management workflows, ensuring that data access is fully aligned with regulatory requirements.
• Collaborate with product teams to map payer and provider data to FHIR resources.
• Troubleshoot and resolve issues with FHIR API integrations, ensuring data integrity and security.
• Work with DevOps teams to deploy FHIR services in cloud environments (AWS, Azure, or GCP).
• Document implementation details, technical designs, and workflows for internal and client-facing use.
Required Skills & Qualifications
• Strong knowledge of FHIR R4 resources (Patient, Coverage, EOB, Clinical, Formulary, Provider Directory, etc.).
• Experience with FHIR Implementation Guides (IGs) such as CMS-0057, CMS- 9115, US Core, CARIN Blue Button, PDex.
• Hands-on experience with authorization frameworks (OAuth2.0, JWTs, PKCE, SMART scopes).
• Familiarity with consent management standards (FHIR Consent resource, ONC/CMS guidelines).
• Solid programming background in languages such as Python, Java, or Node.js.
• Understanding of API security and compliance (HIPAA, PHI/PII data handling).
• Strong knowledge of cloud-based architectures (AWS HealthLake, Azure API Management, etc.).
• Excellent problem-solving, communication, and collaboration skills.
Preferred Qualifications
• Experience with FHIR bulk data APIs and NDJSON pipelines.
• Knowledge of legacy healthcare data formats (HL7 v2, X12 278/837) and mapping to FHIR.
• Prior experience working with payer or provider organizations.
