Position Overview:
Join Bluestone Physician Services as a Team Coordinator! This position plays a critical role within our ADAPT Program serving a group of provider-led teams who provide mobile primary care services to individuals living in long-term residential community settings, such as group homes or assisted living facilities, and who are diagnosed with an intellectual and/or developmental disability, and/or severe and persistent mental illness. The Team Coordinator functions collaboratively with our clinical and non-clinical staff with a primary focus on scheduling activities, route efficiency, team logistics, and acting as a liaison between residential community staff and members of the care team. The team coordinator reports directly to the Program Manager of the ADAPT Program.

Responsibilities:
- Organizing the provider team scheduling and logistics
- Effectively communicate and serve as the key contact for the residential community staff
- Maintain outstanding organizational and customer services abilities
- Effectively communicate with all members of the patient’s care team, both within Bluestone and externally
- Operates office equipment such as computers, phones, and printers to communicate and update electronic medical records and other databases as appropriate.
- Document Management
- Ability to function both independently and collaboratively
- Performs accurate data entry of patient health care information
- Uploads, enters, and assigns patient certifications and recertifications
- Assists with coordination of new patient visits
Location: This role is mostly remote, >90% of the time, with intermittent responsibilities in the field which may include printing and mailing, and/or other new community onboarding activities.
Salary: $23-27/hr. Salary will be commensurate with experience.
Qualifications:
Education/Certification/Experience
- Associate’s degree or higher
- Experience working in the medical field interacting with care providers preferred
Knowledge/Skills/Abilities
- Comfortable navigating electronic medical record systems
- Excellent organization skills and successful track record coordinating, scheduling logistics, managing deadlines
- Excellent interpersonal and customer service skills.
- Excellent written and verbal communication skills.
- Collaborates effectively within team settings with clinical and non-clinical staff, with the ability to assist providers and facilitate an efficient, organized administration flow.
- Detail oriented and accurate.
- Ability to problem solve and work well under pressure.
- Computer proficient
- 55+ WPM typing, knowledge of medical terminology, and understanding of patient care notes.
- Demonstrated compatibility with Bluestone’s mission and operating philosophies.
- Demonstrated ability to read, write, speak, and understand the English language.
