Role Description

We are seeking a dedicated and organized Lead Case Manager to join our on-site team in Kent, WA. This is a full-time role focused on supporting patients throughout their treatment journey by overseeing and managing individualized case plans. The Lead Case Manager collaborates with patients, providers, and other team members, facilitates care coordination, and ensures access to essential resources. Additionally, this role involves supervising a team of case managers, mentoring staff, and providing strategies for enhancing patient and organizational outcomes.

Qualifications
  • Strong organizational and case management skills, including experience in creating, implementing, and monitoring treatment or service plans
  • Proficiency in client advocacy, care coordination, and navigating community resources
  • Strong communication and interpersonal skills to work with patients, multidisciplinary teams, and external stakeholders positively and effectively
  • Team leadership and mentoring capabilities, including experience supervising staff or leading case management teams
  • Familiarity with addiction treatment methodologies, behavioral health best practices, and recovery resources
  • Ability to maintain accurate patient records and documentation while ensuring confidentiality
  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field; relevant certifications or clinical licensure preferred
  • Previous experience in substance use treatment, mental health, or healthcare settings is a plus
