Patient Administrative Coordinator

Position Summary:

As a Patient Administrative Coordinator (PAC), you are the vital link between patients, providers, and care teams, helping ensure every interaction is smooth, timely, and compassionate. In this role, you’ll combine strong administrative skills with patient coordination expertise to keep patients healthy and supported in their homes. From scheduling appointments and managing records to facilitating communication across multiple stakeholders, you’ll play a key part in delivering an exceptional care experience. This position offers variety, working in-office, virtually, and occasionally in patients’ homes, while building meaningful relationships with patients and families. Job Description:

Primary Responsibilities

  • Engage patients through phone, text/portal, video, and in-person outreach to share program details and complete intake steps.
  • Schedule and confirm appointments with clinicians and external providers; arrange interpreter services when needed.
  • Coordinate delegated RN orders and ensure timely fulfillment.
  • Staff the medical office 1–2 days per week; monitor and replenish supplies; assist with inventory checks alongside the clinical lead.
  • Send enrollment, transition, and completion notifications to care teams; manage messages in the EMR in-basket.
  • Securely transmit post-visit notes and records to primary care offices and confirm receipt.
  • Serve as a liaison among patients, families, providers, and internal teams to resolve issues and ensure clear next steps.
  • Support care team during in-home visits as needed.
  • Verify eligibility and benefits; update demographics and insurance; initiate or coordinate prior authorizations.
  • Assist with medical record requests, retrieval, indexing, and secure storage; track turnaround times.
  • Compile and distribute reports for leadership and clinical teams.
  • Support projects focused on documentation quality and data integrity.
  • Prepare and edit correspondence, reports, memos, and meeting minutes; maintain accurate documentation in EMR and CRM systems.
  • Perform general administrative tasks (mail, scanning, faxing, copying, data entry); troubleshoot routine office equipment issues.
  • Adhere to HIPAA and all privacy, security, and compliance requirements.
  • Follow standard workflows, contribute to process improvements, and escalate issues promptly.
  • Participate in training, huddles, and quality initiatives; meet productivity and service standards.
  • Other duties as assigned.

Qualifications

  • Education: High school diploma or GED required; associate’s degree or relevant coursework preferred.
  • Experience: Minimum 2 years of administrative support in a healthcare setting; experience with patient coordination, scheduling, or front office operations preferred.
  • Technical Skills: Proficiency in MS Word, Excel, and Outlook; experience with EMR systems (Epic preferred).
  • Communication: Strong written and verbal skills; professional phone etiquette; ability to explain processes clearly.
  • Organization & Detail: Exceptional attention to detail and accuracy; strong time management and follow-through.
  • Problem-Solving: Resourceful with sound judgment; able to identify barriers, troubleshoot, and escalate appropriately.
  • Additional: Familiarity with medical terminology and referrals; bilingual skills a plus.
  • Comfortable learning and using multiple computer systems; experience with practice or relationship management systems preferred.
  • Flexible and adaptable in a dynamic environment.
  • Must have own reliable transportation for travel to patient homes and office as required.

Pay Transparency:

Factors that may be used to determine your actual pay rate include your specific skills, experience, qualifications, location, and comparison to other employees already in this role. In addition to the base salary, certain roles may qualify for a performance-based incentive and/or equity, with eligibility depending on the position. These rewards are based on a combination of company performance and individual achievements.

The hiring range for this position is:

$39,000.00-$48,150.00
