The Manager, Chronic Special Needs Program (CSNP) will direct and lead the operations of the CSNP and supervise assigned staff. This role will be responsible for overseeing the program development, priorities and supporting health management and quality improvement initiatives for members with chronic conditions. This role will monitor performance metrics, productivity, patient experience, company key performance indicators and compliance with regulations and department policies.
This role is a fully remote position, applicable state licensure may be required.
Responsibilities
Manage and lead the chronic special needs program.
Identify innovative and strategic ways to improve the healthcare of CSNP patient population while reducing the cost of care.
Oversees and collaborate in the development of care and processes, policies, and practice standards as it relates to the CSNP team.
Assist with the continued development and creation of care management workflow within Renal Care to achieve best practice workflows and standardization.
Collaborate with the Quality assurance audit and training team to drive continuous improvements in quality performance.
Maintain compliance with organizational policies, procedure, evidence-based guidelines, and NCQA (National Committee for Quality Assurance) compliance and regulatory requirements.
Communicate with leadership to support care management initiatives, dashboard metrics, and key performance indicators.
Support training, re-training and ongoing training efforts and identify gaps where additional staff training/coaching is needed for the CSNP team.
Build and foster relationships with cross functional teams and other stakeholders, including health plans.
Develop annual goals and support team growth and development through continuous training.
Cultivate and promote a mission-driven culture of high-quality performance, with a member focused on customer service, consistency, and accountability

Qualifications
Required
- Bachelor’s degree in nursing from an accredited school with current RN licensure
- Minimum 5 years of clinical experience, preferably in population health, community public health, case management, or coordinating care across multiple settings and with multiple providers.
- Minimum 3 years of management experience.
- Experience creating and developing clinical programs and improvements.
- Experience working in a fast-paced, dynamic environment while maintaining focus on key goals.
Preferred
- A master’s degree or another advanced degree in nursing, business, health services research, health policy, or other relevant fields preferred.
- Experience with care management competencies.
- Experience working with vulnerable populations.
- Certification in Case Management.

Strong project management skills with the ability to manage change and positively influence others.
Effective written and verbal communication.
Ability to navigate ambiguity with the aid of structured problem-solving techniques.
