The Senior Director of Value-Based Care (VBC) is a strategic and influential leader responsible for cultivating long-term, high-impact partnerships with physicians, healthcare providers, and health systems. This role drives measurable improvements in both financial and clinical performance across the continuum of care, with a focus on transforming outcomes for patients with CKD and ESRD.

Operating at the intersection of strategy, operations, and relationship management, the ideal candidate will have what it takes to not only set the vision, but to also lead execution for market-level VBC initiatives. They apply deep expertise in value-based care models to solve complex business challenges, shape provider engagement strategies, and align cross-functional teams around shared goals. With a strong command of data, contracts, and care delivery innovation, this leader plays a critical role in advancing Somatus’ mission to improve quality of life for patients while delivering sustainable value to partners.


Responsibilities

  • Primary relationship owner with medical practices in the region, responsible for fostering positive relationships and support to VBC group as primary relationship owner with contracted medical practices in the region.
  • Interface with and engage providers strategically prioritized by market data segmentation
  • Report the health of VBC partnership relationships to leadership; implement strategies for remediation, mitigation, and improvement as needed
  • Support, manage, and maintain existing provider relationships and assist with the sourcing of agreements for new VBC provider relationships
  • Foster formal business relationships with key provider stakeholders within the clinically integrated networks - including Nephrologists, PCPs, Specialists, SNFs, Dialysis Centers, Hospital Systems, and ASCs - to support Somatus’ quality and financial objectives in the market
  • Collaborate with Somatus’ payor partner market teams to ensure alignment with provider engagement strategy and tactics
  • Develop provider quality and financial incentives and contractual relationships with market providers in connection with wider Somatus provider engagement and operational team
  • Overall knowledge of Profit & Loss (P&L) business management (pricing, financial reporting, census measures, variance reports, labor reports, etc.) in a healthcare setting
  • Collaborate with Population Health leadership on forecasting and aligning staffing needs
  • Maintain documentation on provider engagement and network efforts, including contractual documents and CRM inputs if needed
  • Support development of provider scorecards with Somatus’ Product and Innovation team
  • Partner where necessary with Somatus’ clinical managers to support care team objectives related to network goals
  • Show proficiency in the Somatus RenalIQ application and speak to qualitative data outputs in provider facing meetings

Qualifications

Required

  • Bachelor’s Degree Preferred (Business, Healthcare Administration, Public Health)
  • 5-7 years’ minimum experience working across population health, integrated health system, or provider relations, with a strong understanding of value-based care models
  • Skilled at engaging and influencing a range of provider types, including Nephrologists, PCPs, Specialists, and Hospital Systems
  • Experience supporting and advancing quality initiatives
  • Robust planning, preparation, and presentation capabilities, with working knowledge of reimbursement, health regulations, and bonus methodologies
  • Demonstrated executive presence in conducting meetings and presentations with Physicians and key stakeholders
  • Self-directed and effective in a remote work environment, with the ability to manage priorities independently
  • Demonstrates a collaborative, inclusive, and highly organized leadership style focused on driving measurable outcomes, while fostering a team-oriented culture and cultivating strong cross-functional relationships
  • Ability to travel 75% (overnight travel required) throughout the state/regions and travel to HQ (Tyson’s Corner, VA) as needed

Preferred

  • Master’s Degree Preferred (Business, Healthcare Administration, Public Health)
  • Understanding of Value Based Contracting / Pay for Performance / Healthcare Trends
  • Demonstrated ability to manage multiple projects and meet deadlines
  • Comprehensive knowledge of all Microsoft Office applications
  • Ability to utilize a CRM salesforce tracking tool
  • Comprehensive knowledge of Medicare, Medicaid and commercial policies, processes and procedures
  • Cross departmental collaboration with briefings and area meetings; maintains frequent contact with department team
