Job Title & Role Description:

The Manager, Nursing Operations is responsible for leading a team of remote and field-based nurses, ensuring high-quality care delivery and operational excellence across all Upward Health markets. This role focuses on optimizing workflows, driving performance through data analytics, and implementing best practices to enhance patient outcomes. The Manager collaborates with internal and external partners to improve care coordination, streamline processes, and maintain compliance with healthcare regulations. Success in this role requires strong leadership, strategic problem-solving, and the ability to drive continuous improvement in a fast-paced, evolving healthcare environment.

This position requires a strong clinical background, care management experience, and expertise in data analytics, process improvement, and regulatory compliance. The ideal candidate is a proactive leader who thrives in a fast-paced, high-pressure environment and is passionate about driving operational excellence in nursing care delivery.

Skills Required:

Required:

  • Registered Nurse (RN) with an active license in a compact state (or ability to obtain licensure in multiple states).
  • 5+ years of nursing experience.
  • 3+ years in a leadership role overseeing clinical teams in a healthcare setting.
  • Strong background in care transitions, discharge planning, and readmission prevention strategies.
  • Expertise in data analytics, performance measurement, and process improvement methodologies.
  • Proficiency in electronic health records (EHR).
  • Understanding of healthcare regulations, payer requirements, and compliance standards (URAC, NCQA, Medicare/Medicaid guidelines).
  • Exceptional leadership, communication, and conflict resolution skills.

Preferred:

  • BSN or MSN degree (preferred but not required).
  • Certified Case Manager (CCM) or equivalent certification.
  • Experience in value-based care models and population health management.
  • Knowledge of social determinants of health (SDOH) and community-based care coordination.
