ABOUT THE ROLE

As a Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure that optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your role is essential in improving patients' quality of life by delivering care over the phone and through virtual platforms.

WHAT YOU’LL DO

In this role, you will provide direct patient care through virtual platforms and deliver high-quality care to meet each patient's unique needs. You’ll provide comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care, offering clinical and care coordination as needed. On a day to day basis, this may include:

  • Clinical Support: You’ll provide consultative support over the phone, email, or video and be available for urgent clinical escalations when required.
  • Symptom Triage: You will triage the responses to our Symptom Survey as well as all other inbound reported symptoms, supporting the patient yourself where possible and escalating to a Nurse Practitioner as needed.
  • Care Navigation: You will proactively identify and address additional patient needs, including care coordination, nutritional support, discharge planning, and provider referrals.
  • Patient Education: For patients who are new to either their diagnosis or Daymark, you’ll help them understand their diagnosis, Daymark Health’s care offerings, and the best next steps to take.
  • Relationship Building: You will establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.
  • Integrated Care Coordination: You’ll work closely with the rest of the clinical team supporting Daymark patients (including Nurse Practitioners, Medical Directors, and Social Workers) to ensure top-quality care and alignment on clinical protocols. You’ll also collaborate with non-clinical Health Partners to address social determinants of health, such as food security and transportation access.
  • Operational Excellence: You’ll adhere to Daymark’s policies, procedures, and documentation standards, collaborating with the Clinical Operations team to ensure the most effective use of our systems, tools, and technology.

WHAT WE ARE LOOKING FOR:

  • Experience: A Bachelor of Science Degree in Nursing is required, along with a compact unrestricted registered nurse (RN) license (or ability to obtain) and licensure in Rhode Island (or are willing to be licensed in RI). Additionally, you have at least 3 years of nursing experience, including 2 years in oncology nursing. You are passionate about helping patients understand the benefits of palliative care.
  • Qualifications: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are strongly preferred or will be obtained within 2 years of hire.
  • Comfort with Technology. You’re comfortable using Slack, Zoom, Google Suite, and EHR systems.
  • Strong relational skills. You have an uncanny ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Bias to action: You don’t wait for someone to tell you what to do. You know how to identify and prioritize your initiative’s needs and do what it takes to ensure that urgent and important needs are acted on immediately.

ADDITIONAL INFORMATION

This is a full time, remote role. The hours are Monday through Friday from 8:30 AM to 5 PM Eastern Time.

Compensation for this role is anticipated to be in the range of $85,000 to $100,000 annually, depending on location and years of experience.
